Here’s is how you create a Google Form with conditional logic.
Start a new form
First, go to Google Forms at https://docs.google.com/forms/u/0/.
Just click on the Blank +.
Give the form a name, ex – Google Form Demo. Provide an optional Form description.
Add your first question
Lets start with a multiple choice question, who are you?
If you click on the six dots just about the question, the options on the right appear. Choose multiple choice.
To add an answer, start typing next to the radio button. And, make the question required.
Add a New Section
Click on the double line icons on the right hand side, at the bottom, just below the video icon.
This will create another section. Call this section Woman.
Create another section, this one called Man.
From this point, you can add addition questions for each section that is specific to either a Woman or a Man.
Go back to the first section where you asked are you a woman or man.
In the bottom right corner, click the More section (those little dots) and choose Go to section based on answer.
For each section, there will now be a dropdown option on the right hand side that lets you choose where to send your visitor based on the answer they provided. For example, if they answer Man, send them to the Man section.
That’s just the tip of the iceberg!
You can continue adding questions and answers that use conditional logic – multiple choice or dropdown works for conditional logic.
Word to the wise … map out what you want to accomplish – what path you want to send your visitors down – before creating the form.
You can style the form a little bit to match the look and feel of your site. Like I said, just a little bit.
